SALARY RANGE:                                  $10.00 HR/24 HOURS WEEK

POSITION SUMMARY

The Casemanage will work with the M3 initiative that is an initiative that supports single female head of households with children under 18, return to college to gain the needed education to enter into a preselected occupation that pays a self-sufficient wage. The Casemanager will work with all the partners involved to ensure the participants success in graduating from college.  The Casemanager is also responsible for daily data entry into the WCA database.

RESPONSIBILITIES INCLUDE THE FOLLOWING:


1.      Plan, schedule, and facilitate meetings with the participants regularly or as needed.
2.      Prepare all reports as required enter all information as required into the WCA data base.
3.      Network with appropriate community sectors and groups for the purpose of recruiting participants.
4.      Attend all informational meetings.
5.      Assist all participants in the applications for any other prequalifying assistance opportunities.
6.      Forecast and work through any barriers that a participant might encounter.
7.      Coach and support participants through application, loan processing, financial and personal management and use of referral services for a positive loan experience.
8.      Attend all group meetings and report out on participants.
9.      Maintain compliance with HIPAA regulations, in regard to accessing Protected Health Information, according to the Table of Access contained in the HIPAA Policy and Procedure Manual.  Employees access is on a “need to know” basis


 

EDUCATION, EXPERIENCE AND/OR SKILLS

1.                  Four year degree in social work or human service field with two years (2) Case management experience.
Or combination of education and experience as listed below:

2.                  Experience working with colleges and college students, traditional or non-traditional.
3.                  Two years (2) Case management experience.
4.                  Computer competent: Microsoft Word, Windows and database.
5.                  Valid PA driver’s license and automobile insurance as mandated by the Agency.
