Institution Name and Project Year: Washington & Jefferson College, Year 2
This in-home care program will be coordinated at Washington & Jefferson College (W&J) and the Community Health Coaches will be W&J student volunteers, trained and supported by Jefferson Hospital of the Allegheny Health Network physicians, and administrators. The Washington & Jefferson College PACC*VISTA will be responsible for developing, enhancing and expanding the Volunteer Community Health Coach Program (VCHCP). The goal of the VCHCP is to support individuals in local communities with personal health and wellness plans upon their dismissal from the hospital and enroll in the Community Care Network. These plans would prevent avoidable and costly hospital readmissions for the patients. The patients who are discharged from hospital services are continually readmitted at high costs to the individuals and those providing care to the patients. There is a community need to improve the health of the local population while at the same time diminishing the costs of healthcare related expenses for the community members who need it most.
