POSITION DESCRIPTION
TITLE:                                                    M3 Casemanager

CLASSIFICATION:                                             Part-time, Non-exempt, Regular (Up to 20 hrs/week)

SALARY RANGE:                                                $ 10.74/hr
 
POSITION SUMMARY
The M3 Casemanager position assists participants in accessing the program’s network of supports and other area resources to help participants overcome barriers which would otherwise prevent participants from successfully completing their college education. At times the casemanager will be working with women who are sometimes going through difficult transitions, being compassionate to their situation and working with them in a positive way will assist participants in overcoming barriers to their success and make good forward strides towards self-sufficiency. Effective communication skills are crucial, as the casemanager also serves as an advocate for participants in the program as well.  The M3 Casemanager will assist the Bridges M3 Coordinator in recruiting community supports and participants for the Mothers Making More program; as well as have the ability to assist in facilitating Economic Stability classes.
RESPONSIBILITIES:
Provide on-going support and guidance by meeting regularly with participants, twice monthly at minimum, to ensure barriers to their success are promptly addressed and minimized.
Assist participants in developing realistic and attainable goals; provide them with feedback and resources necessary to accomplish those goals.
Assist in facilitating Economic Stability classes, and in providing an orientation to participants regarding Ancillary Support Fund guidelines.
Able to work nontraditional hours as assigned by the Bridges/M3 Coordinator.
Reports participant’s progress and concerns to Program Coordinator during regular casemanagement meetings and in monthly reports.
Completes appropriate reports as assigned.
Maintain and keep original case records in compliance with funding source and as prescribed by W.C.A. procedures.
Assist in the preparation of statistical data relevant to the program.
Models appropriate behavior.
Enter all information as required into the WCA data base, CTK, and update it regularly.
Maintain compliance with HIPAA regulations, in regard to accessing Protected Health Information, according to the Table of Access contained in the HIPAA Policy and Procedure Manual.  Employees access is on a “need to know” basis. 
 
SPECIFIC JOB KNOWLEDGE, SKILL AND ABILITY:

Ability to listen effectively.
Ability to communicate verbally and in writing using the English language.

Ability to keep accurate and detailed records and produce reports via agency database.
Ability to function as a team member by assisting coworkers and participants in the successful achievement of program goals and objectives.
 
EDUCATION, EXPERIENCE AND/OR SKILLS:
Four –year degree in social work or human service field or combination of education and experience as listed below:
Minimum of two (2) years group facilitating experience.
Experience working with low-income families and community services.
Two years experience in sales or customer relations.
Two years (2) Casemanagement experience.
Strong written and spoken communication skills, comfortable speaking in front of groups of people.
Strong knowledge of Microsoft Office programs (Word, Excel, PowerPoint), ability to navigate email, comfortable using common internet search engines, ability to learn and utilize agency database efficiently.
Valid PA driver’s license and automobile insurance as mandated by the Agency.
