SUMMARY
As part of a Joint Planning Team, the
Facilitator works to assist adolescents with emotional needs and their families
in strengthening systems of natural support.
This includes improving the ability and confidence of family members in
managing their own services, and organizing formal and community supports through
comprehensive, individualized, holistic support plans. Responsible for carrying out services by
performing the following duties.

RESPONSIBILITIES

- Attends all
mandatory county and state trainings.
- Implements the
10 Principles of High Fidelity Wraparound.
- Implements the 4
Phases of High Fidelity Wraparound.
- Ensures that the
Principles and
Phases of the High Fidelity Wraparound process are delivered with the
highest possible fidelity.
- Seeks and
provides coaching, supervision and professional input when needed.
- Works with youth
and families to create and maintain a team for each adolescent and family.
- Facilitates
family and youth team meetings.
- Ensures that the
family/youth voice and choice is at all meetings.
- Identifies and
links adolescents and families to natural and community resources.
- Tracks graduates
over time.
- Writes, revises
and distributes individualized High Fidelity Wraparound Plans and Joint
Planning Team meeting minutes.
- Works with and
help families to plan and coordinate transitions for the adolescent and
family.
- Manages a
caseload of 8 – 15 families.
- Responds
appropriately and effectively to directives assigned by the Supervisor/Coach.
¨ In meeting the needs
of clients, maintains flexible scheduling availability.
SUPERVISORY RESPONSIBILITIES
This position has no
supervisory responsibilities.
EDUCATION and/or EXPERIENCE
Bachelor's degree in a Human Services or
related field with at least two (2) years of experience in relevant family,
children/youth or community based services. Relevant alternative experience may
substitute for a bachelor’s degree in individual cases (subject to approval
from OMHSAS). Must possess knowledge of
High Fidelity Wraparound Principles and Phases, and maintain HFW training
requirements and credentialing as a FSP.
Must be at least 18 years of age with current Act 33, 34 and FBI
clearances, and current PA state driver’s license with an insurable driving
record. Must possess demonstrated PC
skills including basic MS Office applications.
Additional training or licensing as required by regulations, or as
deemed necessary by the Executive Director or Director.
